Breast reduction (reduction mammaplasty) is a surgical procedure that reduces the size and weight of overly large breasts by removing excess glandular tissue, fat, and skin. The goal is to relieve symptoms such as neck and back pain, skin irritation, and postural problems, while restoring a breast shape that is proportional to the patient's body.

Breast reduction surgery is often recommended for individuals experiencing physical or emotional discomfort due to excessively large breasts (macromastia or gigantomastia). Common indications include chronic neck, back, or shoulder pain caused by the weight of the breasts, as well as skin irritation or rashes (intertrigo) that develop beneath the breast folds. Many patients also suffer from postural problems and find it difficult to engage in physical activities such as sports or exercise. Beyond physical symptoms, psychosocial distress, including lowered self-esteem or social discomfort, can significantly impact quality of life, making breast reduction a valuable option to improve both physical well-being and emotional health.

Anesthesia and Duration

This surgery is typically performed under general anesthesia, ensuring the patient is completely asleep and pain-free throughout the procedure. The surgery usually lasts between 2 to 4 hours, depending on the complexity of the case, the amount of tissue to be removed, and whether additional reshaping or lifting is required to achieve the desired results.

Surgical Techniques

Breast reduction surgery can be performed using different techniques, depending on the amount of tissue to be removed and the desired outcome.

  • Inverted T or Anchor Technique: The most common method, which involves incisions around the areola, vertically down to the breast fold, and along the inframammary crease. This approach allows for significant tissue removal and excellent reshaping.
  • Vertical Scar Technique (Lollipop Method): Includes incisions around the areola and vertically down to the breast crease. It's suitable for moderate reductions and results in less scarring compared to the anchor technique.
  • Periareolar (or Circumareolar) Technique: For minor reductions and mild sagging, with the incision limited to the areolar border.

Important: In all techniques, the nipple-areola complex is preserved along with its blood supply (called the pedicle) and repositioned to a higher position on the breast. In rare cases—such as extreme hypertrophy (gigantomastia) or when blood supply is compromised—a free nipple graft may be necessary.

Proper preparation is essential for a successful procedure:

  • Discontinue smoking, anticoagulants, and NSAIDs
  • Perform mammography or breast ultrasound as needed
  • Preoperative bloodwork, ECG, and anesthesia consultation

Postoperative Recovery

Recovery from breast reduction surgery generally allows a return to work within 1 to 2 weeks, depending on the nature of the job and the individual's healing process. To support healing and reduce swelling, wearing a supportive bra is strongly recommended for 4 to 6 weeks following the procedure. Strenuous activities and heavy lifting should be avoided during this period to prevent complications and ensure proper recovery. While initial improvements are visible early on, the final shape and contour of the breasts typically stabilize over the course of 3 to 6 months as the tissues settle and swelling fully subsides.

Possible Complications

As with any surgical procedure, breast reduction carries certain risks. These may include:

  • The development of a hematoma or seroma—collections of blood or fluid beneath the skin
  • Wound healing issues, particularly in patients with risk factors like smoking or diabetes
  • In rare cases, partial necrosis of the nipple, especially when large amounts of tissue are removed or the blood supply is compromised
  • Loss of sensation in the nipple, which can be temporary or, in some cases, permanent
  • Scarring is inevitable but typically fades over time, becoming less noticeable
  • The ability to breastfeed may be reduced, depending on the surgical technique used and the amount of glandular tissue removed
