The neck lift is a cosmetic surgical procedure designed to improve the appearance of the neck when there is skin laxity, fat accumulation, visible muscle bands, or a poorly defined profile between the neck and jawline.

This procedure can remove excess skin, eliminate or reposition fat under the chin and along the neck, repair the neck muscles (especially the platysma), and redefine the jawline, giving it a more youthful and contoured appearance.

Surgical Techniques

There are various surgical techniques with different degrees of invasiveness:

  • Classic Lift with Skin Undermining: The skin and subcutaneous tissue are elevated and repositioned upward, removing the excess and addressing platysma laxity (sometimes by suturing or partially removing the muscle). The incision usually begins in front of or behind the ear, continues behind the ear, and sometimes ends at the nape.
    • Suitable for:
      • Cases of double chin with abundant skin
    • Recovery:
      • Typically takes 2–4 weeks and may include swelling, mild discomfort, and temporary changes in neck or scalp sensation
  • SMAS Neck Lift (Deep Plane): The most invasive version, where the surgeon works beneath the muscle fascia (SMAS) and may reposition or remove the submandibular gland or part of the digastric muscle. This allows for very natural and long-lasting results by acting on deep anatomical structures.
    • Ideal for:
      • Patients who want significant improvement in both skin and musculature
    • Note:
      • Technically demanding and carries a higher risk of temporary nerve injury (0.2–12%) and hematomas (0.2–4%)
  • Mini Neck Lift (Platysma-only): A less invasive technique often used for mild skin laxity and visible platysma bands. It involves short incisions and focuses on the platysma without extensive skin undermining, resulting in quicker recovery.
    • Note:
      • The outcome is less uniform and not suitable for those with significant fat or skin laxity
  • Neck and Chin Liposuction: For submental fat accumulation (double chin), liposuction is often combined with platysma plasty to improve the contour.
    • Ideal when:
      • Fat is the main issue and skin elasticity is still good
    • Benefits:
      • Minimally invasive, has no extensive incisions, and offers quicker recovery but is not appropriate for advanced cases
  • MACS Neck Lift (Minimal Access Cranial Suspension): A simplified variant of the deep-plane technique, using vertical suspension sutures. Incisions are minimal and usually located in front of the ear, allowing for a shorter recovery.
    • Ideal for:
      • Moderate sagging and patients seeking less invasive correction

Preoperative Preparation

A full preoperative assessment including blood tests, ECG, and anesthesiology evaluation is required. Patients should stop smoking and may need to discontinue NSAIDs or anticoagulants. Surgeon experience and a well-defined surgical plan are essential.

Postoperative Recovery

Postoperative recovery after a neck lift is generally smooth but requires careful attention to ensure optimal healing and aesthetic results.

In the first few days, patients may experience swelling, bruising, and mild discomfort, especially around the jawline and under the chin. A compression garment or bandage is usually worn during this period to reduce swelling and support the neck tissues. It's important to keep the head elevated, including while sleeping, to help minimize fluid accumulation.

Most people are able to return to work within 10 to 14 days, depending on the extent of the procedure and how visible the bruising remains. Strenuous physical activity should be avoided for 3 to 4 weeks, and direct sun exposure to the area should be limited during the healing phase.

Final results typically begin to appear after 4 to 6 weeks, but subtle improvements can continue for several months as the tissues fully settle and any residual swelling resolves.

Possible Complications

As with any surgery, a neck lift carries some risks, though complications are generally uncommon.

  • The most frequent include hematomas, which may require drainage, and infections, typically manageable with antibiotics
  • There is also a small risk of temporary nerve irritation, leading to numbness or weakness in the lower face or neck
  • Other possible issues include poor wound healing, especially in smokers, minor asymmetries, or scarring, which is usually well hidden but can occasionally become raised
  • In rare cases, hair thinning may occur along the incision lines
