Surgical Techniques

Several rhinoplasty techniques are available depending on the patient's goals and anatomy:

  • Aesthetic Rhinoplasty: Aims to improve the appearance of the nose (dorsal hump, tip refinement, nostril symmetry, nose width). The result should be harmonious with the patient's facial features and ethnicity.
  • Functional Rhinoplasty: Targets internal nasal structures (septum, turbinates, valves) to improve breathing. Often combined with septoplasty (correction of the septum).
  • Open Rhinoplasty: An incision is made across the columella (tissue between the nostrils) to lift the nasal skin and allow complete visualization of the internal structures. Ideal for complex cases or revisions.
  • Closed Rhinoplasty: All incisions are made inside the nostrils. No external scar. Shorter operative time and recovery but limited visibility for the surgeon.
  • Preservation Rhinoplasty: An advanced technique that modifies internal structures while preserving the nasal dorsum and ligaments. Minimizes trauma and swelling. Not suitable for all cases.
  • Secondary (Revision) Rhinoplasty: Performed to correct or refine results of previous nasal surgeries. More complex due to scar tissue and altered anatomy.

Preoperative Preparation

Proper preparation is essential for a successful rhinoplasty:

  • Discontinue anticoagulants, NSAIDs, smoking at least 2 weeks before
  • Perform bloodwork, ECG, and clinical evaluation
  • Preoperative photographs are essential for planning

Postoperative Recovery

After rhinoplasty, nasal packing may be used for the first 1 to 2 days to support the internal structure and control bleeding. An external splint is typically worn for about 6 to 8 days to help maintain the new shape of the nose during initial healing. It's common to experience bruising and swelling around the eyes for 1 to 2 weeks, which gradually subsides. Most patients feel well enough to return to work after approximately one week, although residual swelling may persist. The final results of the surgery become fully visible only after 6 to 12 months, as the deeper tissues continue to heal and refine over time.

Possible Complications

As with any surgical procedure, rhinoplasty carries some risks. These may include bleeding and infection, although both are relatively uncommon with proper care. Some patients may experience asymmetry or irregularities in the final shape of the nose, which can occur despite meticulous surgical technique. In rare cases, difficulty breathing may result, particularly if the internal structures are affected—though this is unlikely when the procedure is properly performed. Additionally, about 5 to 15% of patients may eventually require a revision surgery to refine the results or address minor issues that arise during healing.
